APX Provider Agreement

1. Purpose
APX provides procurement coordination services for licensed providers.
2. Independent provider Relationship
The provider operates independently and is not an employee or agent of APX.
3. No Medical Services by APX
APX does not practice medicine, diagnose, prescribe, or provide clinical direction.
4. Product Procurement & Use
The provider assumes full responsibility for product selection, use, and patient application.
5. Regulatory Compliance
The provider agrees to comply with all applicable laws and regulations.
6. No Reliance on APX
The provider does not rely on APX for medical or legal guidance.
7. Indemnification
The provider agrees to indemnify and hold APX harmless from any claims related to product use, patient outcomes, or regulatory violations.
8. Limitation of Liability
APX is not liable for any damages, clinical outcomes, or provider actions.
9. Term & Termination
This agreement remains in effect upon use of APX services.
10. Governing Law
This agreement is governed by the laws of Texas.
11. Acknowledgment
By using APX services, the provider agrees to these terms.
